2013 Charity Golf Tournament 8/23/2013

Please join us at the Rattlesnake Fire Special Funds 2013 charity golf tournament to benefit Elbert County community local charities. It is at the Spring Valley Golf Club on Friday August 23. There is a 8:00am Shotgun Start in a scramble format. It is $100.00 per person or 375.00 per foursome. This includes green fees, cart, range balls, and food!

Hole sponsors at the $500.00 level includes sponsor sign at hole, golf foursome, cart range balls, and food. Or the $250.00 level will get you a sponsor sign at a hole and food.

Prizes for men's and women's longest drive, closest to the pin, first and second place foursome, highest score and miscellaneous prize drawings.

Food will be a burger and brat buffet!

The registration deadline is Sunday, August 4th, 2013.

Scholarship Guidelines and Application

The following guidelines and application for the Rattlesnake Fire Special Funds Scholarship must be completed in full to be considered for award of scholarship. This scholarship is being awarded for students that plan on attending courses pertaining to the Fire, EMS or Medical fields. Scholarship will be awarded to students attending accredited schools or programs only. Students can attend in-state or out of state schools.
Guidelines:
1. Scholarship is open to all current residents of Elbert County. Applicants must provide proof of Residency.
2. Immediate family (spouses, children or grandchildren) of current or past Rattlesnake Fire Protection District in good standing may apply.
3. Current and past member of Rattlesnake Fire Protection District, resident and Out of District Program are excluded.
4. Scholarship amount(s) depend on funds available. Typically 2-$1000 scholarships will be awarded.
5. Scholarship winners agree to have their names published in the Rattlesnake Rattler newsletter and/or other Elbert County news sources.
6. Application:
a. Applicants must have a GPA of 3.0 or better. Transcripts must be submitted with application, high school or college.
b. Applicants must be enrolled in an accredited school or an accredited program.
c. If enrolled in an accredited University, College or Trade School, applicant must be enrolled as a full time student.
d. Applicant must submit a letter or other proof of acceptance for the accredited institution they will attend.
e. Applicants must submit 2 letters of recommendation. Letters cannot be from immediate family. Letters should state why applicant is deserving of this scholarship.
f. Applicants must submit a letter requesting this scholarship. Letter should include the student’s goals, career plans, strong points and purpose for continuing their education.
g. This Scholarship is not transferable

All application information will become property of Rattlesnake Special Funds Scholarship Committee. No information will be returned. All information will be kept strictly confidential. Scholarship is awarded on a yearly basis if funds are available. Applicants must re-apply to be considered for additional scholarships.

The decision of the Rattlesnake Fire Special Funds Scholarship Committee will be final. All applications must be post marked no later than June 1 of current year. Rattlesnake Fire Special Fund

The Rattlesnake Fire Special Fund is a 501(c)3 non-profit organization comprised of and managed by the members of Rattlesnake Fire Department. Our function is to provide public awareness and education, support local charities, provide community service and assist resident of the Rattlesnake Fire Protection District and Elbert County who have special needs. Rattlesnake Fire Special Fund has made available two, one thousand dollar scholarships for qualified residents of the Rattlesnake Fire Protection District. Low cost Heart-Saver CPR classes are held, for the public, on a quarterly basis.

We host several annual events to raise funds and provide service to the community:
a Veterinary Clinic, provides low cost vaccinations for dogs and cats; our Charity Golf Tournament, whose proceeds go to assist local charities; the Harvest Festival, at Station 251, offers public education, and a safe alternative to trick or treating; Pictures With Santa, where the public are invited to visit Station 252, participate in activities for the children, take a photo with Santa, and whose proceeds are donated to local Childrens Hospitals and a Santa Drive, our Engine 253 provides transportation for Santa to visit a local assisted living facility, and tour the community, handing out candy canes.

The hard working members of Rattlesnake Fire Department are dedicated to serving our community and Special Fund provides us with various ways to offer that service.

Medical Records

Jan 1, 1983
Patients shall have access to their own medical records. The district will provide copies of these records for a fee of $ 0.25 per page copied provided the patient has come to us in person and has photo identification verifying they are patient. This identification shall be photocopied and a copy of it along with a dated explanation of what was released shall be attached to the original medical record on file. If the patient is a minor, the parents or legal guardians of the patient shall provide sufficient identification to allow the release of copies of the medical records.

Individuals with Power of Attorney for a patient shall have access to the patients medical records. The district shall provide copies of these records at $0.25 per page copied provided the individual with the Power of Attorney has come to us in person and has photo identification verifying they are the correct person. This identification shall be photo copied and a copy of it along with a dated explanation of the medical record release and a copy of the Power of Attorney document shall be attached to the original medical record file.

Medical records may be mailed if the district receives a document to release medical information signed by the patient and notarized by a notary public verifying it is the patient that signed the release of medical information form. This form shall be attached to the original medical record and filed. A fee of $ 0.25 per page copied and shipping (certified mail) will be charged for mailing medical records.
